. Any and all information collected on this site will
be kept strictly confidential and will not be disclosed. Your
information will not be used in ways that you have not consented

The
use of Triad Medical Supply website is governed by the policies,
terms and condition set forth below. By using this Site or
submitting an order for products or services, you indicate your
acceptance of, and agreement to be bound by, the terms and
conditions set forth by this website. If you do not agree to these
terms and conditions, please do not use this Site and please do not
submit any orders for products or services. Terms and
conditions may be changed by Triad Medical Supply at any time in
the future. All contents included in this website are
protected by U.S.
and international trademarks and copyright laws. The contents
of this website, including the
reproduction, modification, distribution, transmission,
republication, display or performance, of the content on this site
is strictly prohibited.
All
prices, policies, terms and condition are subject to change without
prior notification. In the event a product is listed at
an incorrect price due to typographical, photographic, technical
error or human error in pricing information received from our
suppliers, Triad Medical Supply shall have the right to refuse or
cancel any orders placed for product listed at the incorrect
price. Triad Medical Supply shall have the right to refuse or
cancel any orders placed on mis-priced
products whether or not the order has been confirmed and your
credit card charged. If your credit card has already been charged
for the purchase and your order is canceled, Triad Medical Supply
shall immediately issue a credit to your credit card account in the
amount of the incorrect price.
Triad
Medical Supply shall automatically charge and withhold the
applicable sales tax for orders to be delivered to addresses within
California,
unless the customer is tax exempt and provides the required tax
exempt information to Triad Medical Supply before ordering. Each
customer shall be solely responsible for all sales taxes, or other
taxes, on orders shipped to any other state.
An order cancellation fee of 10% will be charged on all
orders that have already been processed, but not shipped.
Freight Charges
Please
Note: To keep shipping costs at a minimum Triad Medical Supply ships
all packages under 1lb. USPS. USPS does not provide Tracking
Numbers. Some orders may require additional freight charges, due to
the size, weight of products ordered and/or the shipping location.
If your order is affected by one of these conditions, you will be
contacted and given the option of accepting the additional charges,
choosing another shipping method (when available) or canceling the
order. Products shipped directed from the manufacturer may required
additional shipping time. Triad Medical Supply is not liable for
shipments made to us. We recommend that our customers
use shipping companies such as UPS, FedEx, DHL/ Airborne
Express or the U.S. Postal Service with good packaging (original
packing preferred) and full insurance paid. The RA Number that was
issued to you should be clearly written on the top of the box and
not covered by any labels. Some of the products will be shipped
directly to the manufacturer. Please contact us for proper shipping
instructions.
Most
orders received by 12:00pm PST will be shipped the same day.
Also, we ship only to physical addresses within the Continental
USA, not to P.O. Boxes, APO's and FPO's.

Additional Freight
Charges May Apply
Large Quantities Purchases & Over Weight and Over
Sized Purchases
On large quantities purchases
and over weight and over sized purchases, Triad Medical Supply
reserves the right to charge actual freight charges. These charges
would appear as a separate credit card charge. You will be notified
prior to shipment if this is necessary.
Returns
Policy
Canceled orders after it has
been shipped will be considered a return under this policy.
Products should be carefully
examined. Any shortages or error claims in shipment must be made
within 48 hours of receiving the merchandise.
Items returned must be in
resalable condition and in its original packaging with instructions
and warranties. Non-resalable items received will not be
returned.
All returns must have a RA
Number (Return Authorization). The RA Number must be obtained
by calling our toll free number, 888-80-TRIAD between 9:30am &
5:00pm PST, Monday - Friday. The RA Number must be written on
top and side of the shipping package. Items being returned
must be received by Triad Medical Supply within 7days of receiving
the Return Authorization Number. Returned packages must be
returned Triad Medical Supply, 5208 Jackson Drive, Ste. 110, La Mesa, Ca.
91941. Customer will pay for all shipping expenses for
returned items.
Damaged packages must be noted
on the bill of lading prior to signing for the packages.
Concealed damage must be reported to the freight company within 10
days of receipt of merchandise. All freight damage claims
must be reported to the freight company.
Restocking Fees:

Non-Returnable Policy
Special Orders merchandise are not returnable. Also, worn products,
cushions, pillows, beds, mattresses, wheelchairs, motorized
wheelchairs, scooters, patient lifts and bathroom products are not
returnable and refundable for hygienic reasons.
